KOSHA Flourish Membership Consent & Terms of Service

By enrolling in the KOSHA Flourish Membership, you acknowledge that you have read, understood, and agreed to the following terms.

1. Nature of the Membership

KOSHA Flourish is an educational and physician-guided longevity membership.

This membership provides:

Group-based educational sessions
Hormone and metabolic health education
Lifestyle and food-based physiology guidance
Seasonal implementation cycles
Access to physician-ordered laboratory testing (in licensed states only)
Optional access to 1:1 consultations at preferred member pricing

The membership itself is not a substitute for individualized medical care.

Enrollment does not automatically establish a formal physician-patient relationship unless and until a separate individual medical consultation is scheduled and consented to.

2. Educational Scope

Group sessions and written materials are educational in nature.

They are intended to provide general information about:

Hormone physiology
Metabolic health
Circadian biology
Detoxification pathways
Nutrition strategies
Genomic principles

Educational content is not individualized diagnosis or treatment.

Medical recommendations require a formal consultation and clinical evaluation.

3. Physician-Ordered Laboratory Testing

Where Dr. Mimansa is licensed to practice medicine, members may request physician-ordered laboratory testing.

Laboratory testing:

Is optional
May require additional consent
Is billed separately
Does not guarantee individualized interpretation without a scheduled consultation

Lab ordering availability depends on state licensure and regulatory requirements.

Members outside licensed states may not access physician-ordered testing through this membership.

4. Hormone Assessment & Management

Hormone education is included in the membership.

If a member elects to pursue individualized hormone management:

A separate 1:1 consultation must be scheduled
A physician-patient relationship will be formally established
Additional informed consent will be required
Treatment decisions are made at the sole clinical discretion of the physician

Hormone prescriptions are not included in the membership fee.

5. Genomic & Advanced Offerings

Members may access advanced services such as:

Genomic Blueprint testing
Precision biomarker panels
Targeted supplementation programs

These services are separate from the monthly membership fee and require additional consent and payment.

Genomic interpretation is informational and does not replace medical diagnosis.

6. Payment Terms

Membership Fee: $299 per month

Payment is billed automatically on a recurring monthly basis.
By enrolling, you authorize automatic recurring charges.
Membership may be canceled at any time prior to the next billing cycle.
No partial refunds are issued for mid-cycle cancellations.

Failure of payment may result in suspension of membership access.

7. Cancellation Policy

Members may cancel at any time.

Cancellation must be submitted at least 48 hours prior to the next billing date to avoid automatic renewal.

No refunds are issued for completed billing periods.

8. No Emergency Services

KOSHA Flourish does not provide emergency medical services.

If you are experiencing a medical emergency, call 911 or seek immediate emergency care.

The membership platform should not be used for urgent medical concerns.

9. No Guarantee of Outcomes

Individual health outcomes vary.

Participation in this membership does not guarantee:

Weight loss
Hormonal normalization
Symptom resolution
Disease prevention

Health outcomes depend on numerous individual factors including compliance, genetics, lifestyle, and underlying medical conditions.

10. Personal Responsibility

By enrolling, you acknowledge that:

You are responsible for your own health decisions
You will consult your primary care physician or specialist as appropriate
You will disclose relevant medical history during any private consultations

Participation is voluntary.

11. Confidentiality

Group sessions are conducted in a shared environment.

While MIMANSA maintains professional confidentiality standards, other participants are responsible for maintaining discretion regarding shared discussions.

Members agree not to record sessions without written permission.
